How to make a family introduction

Introduction Criteria
Services are eligible to all families living in Wiltshire with a child aged 0-19 (or 25 with SEND).
Families at Child in Need or above are not eligible for one to one outreach support (ie support in the home or via one to one video calls). However, we can work with families at Early Support Assessment level and can be the Lead Practitioner on for these assessments (if it is appropriate for us to do so).
All families are eligible to be introduced to our other services (eg parenting, domestic abuse groups (provided they are no longer in an abusive relationship) and literacy groups etc).
Outreach Introductions
If you would like to introduce a family for one to one outreach, please complete our outreach introduction form below. There are two versions available, one which can be printed and completed manually, and one which can be completed on your device and emailed to us.
When you return the form to us, please ensure that this is password protected. The password should be sent separately to referrals@spurgeons.org.
Once we have received your introduction you will get a receipt straight away to let you know that it is safely with us. Introductions are managed on a daily basis and you will be allocated a worker within two weeks of us receiving your introduction (unless we are operating a waiting list).

Other Family Hub services
Parenting
If you have a family who would like to take part in one of our parenting programmes, you can either add them to an expressions of interest, or the family can sign up themselves. The links are all on our parenting pages parenting programmes.
Community navigators
There is no introduction necessary for this service. Families just need to come into the hub and spoke buildings on days that we are present. A full list of the venues, dates and times can be found by visiting Family Navigator Locations.

Baby Bundle Introductions
For families struggling to buy clothes and other items for the baby (0 to 12 months) our Together for Families colleagues provide free bundles of the most commonly required items.
